Het is niet duidelijk of onder patiënten met aanvankelijk niet-resectabel squameus celcarcinoom van de slokdarm (iuESCC) inductie-immuunchemotherapie (iIC) resulteert in betere conversiepercentages dan inductie-chemotherapie (iC). Een retrospectieve cohortstudie in vier centra in China heeft iIC en iC in deze setting vergeleken. Prof. Guibin Qiao (Zuidelijke Medische Universiteit, Guangzhou) en collega’s publiceren de studie in Annals of Surgical Oncology.¹

De studie includeerde 309 iuESCC-patiënten, onder wie 150 iIC kregen en 159 iC. Conversie naar resectabiliteit werd bereikt in 127 van 150 patiënten in de iIC-groep en 79 van 159 patiënten in de iC-groep (84,7% versus 49,7%; p<0,001). Pathologisch complete respons werd bereikt in 22,0% versus 5,1% (p=0,001). De mediane overall survival werd niet-bereikt in de iIC-groep en was 36,3 maanden in de iC-groep. Significante prognostische factoren voor overall survival in multivariate analyse waren iIC versus iC (HR 0,215; p<0,001) en resectie (wel versus niet HR 0,262; p<0,001).

De onderzoekers concluderen dat onder patiënten met aanvankelijk niet-resectabel squameus celcarcinoom van de slokdarm, inductie-immuunchemotherapie resulteerde in hoger percentage conversie naar resectabiliteit en langer overall survival dan inductie-chemotherapie.
