Chirurgie voor lokaal-gevorderd larynx- of hypofarynxcarcinoom kan ongunstige consequenties hebben voor de kwaliteit van leven van de patiënten. Een fase 2-studie van Fudan Universiteit (Shanghai, China) heeft inductietherapie met toripalimab (anti-PD-1) in combinatie met chemotherapie voor orgaanbehoud in deze patiënten geëvalueerd. Prof. Yu Wang en collega’s publiceren de studie in Clinical Cancer Research.¹

De studie includeerde patiënten met resectabel lokaal-gevorderd squameus celcarcinoom van larynx of hypofarynx en een ECOG performance status 0 of 1. De patiënten kregen drie cycli inductiechemotherapie plus toripalimab. Patiënten met complete of partiële respons op inductiechemotherapie kregen concurrente chemoradiotherapie gevolgd door toripalimab-onderhoud. Patiënten zonder respons ondergingen chirurgie gevolgd door adjuvante (chemo)radiotherapie en toripalimab-onderhoud. Het primaire eindpunt was larynx-behoud drie maanden na de radiotherapie.

De studie includeerde 27 patiënten (22 stadium IV). De ORR op de inductietherapie was 85,2%. Het percentage met behouden larynx drie maanden na de radiotherapie was 88,9%. De mediane follow-up was 18,7 maanden. De één-jaars percentages voor OS, PFS, en larynxbehoud waren 84,7%; 77,6%; en 88,7%. Na exclusie van de vijf patiënten die pretreatment tracheostomie hadden ondergaan was het één-jaars percentage van larynxbehoud 95,5%.

De onderzoekers concluderen dat inductietherapie met toripalimab en chemotherapie bemoedigende activiteit had en resulteerde in een veelbelovend percentage patiënten larynxbehoud.
