Toevoeging van PD-1 remmers aan chemotherapie heeft geresulteerd in verbetering van uitkomsten van geselecteerde patiëntenpopulaties met metastatisch oesofagogastrisch adenocarcinoom (EGA). De gerandomiseerde multicenter fase 2-studie AIO INTEGA in Duitsland heeft nivolumab-trastuzumab gecombineerd met ipilimumab of mFOLFOX6-chemotherapie voor niet-eerder behandeld gevorderd HER2-positief EGA geëvalueerd. Prof. Alexander Stein (Universiteit van Hamburg-Eppendorf) en collega’s publiceren de studie in JAMA Oncology.¹

INTEGA werd uitgevoerd in 21 centra. De studie randomiseerde 88 patiënten naar nivolumab-trastuzumab-mFOLFOX6 (FOLFOX-groep; n=44) of nivolumab-trastuzumab-ipilimumab (ipilimumabgroep; n=44). Het primaire eindpunt was twaalf-maands overall survival percentage in beide armen vergeleken met historische controle (trastuzumab-chemotherapie, resulterend in twaalf-maands OS-percentage 55%). Twaalf-maands OS was 70% (95%-bti 54-81) in de FOLFOX-groep versus 57% (41-71) in de ipilimumabgroep. Graad 3 of hoger en ernstige adverse events werden gezien in 29 respectievelijk 15 patiënten in de FOLFOX-groep en in 20 respectievelijk 17 patiënten in de ipilimumabgroep, met hogere incidentie van neuropathie met FOLFOX en immuungerelateerde AEs met iipilimumab.

De onderzoekers concluderen dat de combinatie van trastuzumab, nivolumab en FOLFOX resulteerde in gunstige overlevingsresultaten en respons vergeleken met historische controle. Dit werd niet gezien met de combinatie van trastuzumab, nivolumab, en ipilimumab (visual abstract).

Summary: The multicenter randomized phase 2 AIO INTEGA trial compared nivolumab plus trastuzumab combined with ipilimumab versus mFOLFOX6 chemotherapy for newly diagnosed advanced HER2-positive esophagogastric adenocarcinoma. The chemotherapy group had favorable survival and response compared with historical control. This was not seen in the ipilimumab group.
