In
vroeg-stadium EGFR-gemuteerd
niet-kleincellig longcarcinoom is chirurgie de primaire behandeling, en zijn er
geen op individuele patiënten toegesneden adjuvante behandelingen bekend. Een
multicenter retrospectieve studie in Zuid-Korea heeft risicofactoren voor
recidiefvrije overleving na resectie in deze setting geïnventariseerd. Prof.
Keunchil Park (Samsung Medisch Centrum, Seoel) en collega’s publiceren de studie in Clinical Cancer Research.1
De studie
includeerde 1181 patiënten met pathologisch stadium IB tot en met IIIA
niet-squameus EGFR-gemuteerd NSCLC
die tussen begin 2008 en september 2020 curatieve chirurgie ondergingen. De
mediane follow-up was 38,8 maanden (range 0,5-156,2). De mediane RFS was 73,5
maanden (95%-bti 62,1-84,9) in de groep patiënten met stadium IB; 48,7 maanden
(41,2-56,3) in de groep met stadium II; en 22,7 maanden (19,4-26,0) in de groep
met stadium IIIA (p<0,001). Klinisch-pathologische factoren die in
multivariate analyse geassocieerd waren met RFS waren pathologisch stadium,
micropapillair subtype, vasculaire invasie, pleurale invasie, en pathologische
klasse naar cell of origin (type II pneumocyte-like tumor cell versus bronchial surface epithelial cell-like tumor
cell). Moleculaire risicofactoren voor slechte RFS onafhankelijk van
p-stadium waren non-terminal respiratory unit (non-TRU) van het RNA subtype (HR
3,49; p<0,01) en TP53-mutatie HR
2,50; p=0,01). Onder de patiënten met recidief was de progressievrije
overleving met EGFR-TKI slechter in de groep patiënten met
APOBEC-mutatiesignatuur dan in de patiënten zonder dit signatuur (8,6 versus
28,8 maanden; HR 4,16; p=0,02).
De
onderzoekers concluderen dat patiënten in de laag-risicogroep met TRU-subtype
en TP53-wildtype zonder
klinisch-pathologische risicofactoren wellicht geen adjuvante EGFR-TKIs nodig
hebben. Patiënten in de hoog-risicogroep met non-TRU subtype en/of TP53-mutatie dient een nieuwe adjuvante
strategie van EGFR-TKI plus chemotherapie of antiangiogene middelen te worden
onderzocht. Voor patiënten met het APOBEC-mutatiesignatuur is een alternatieve
adjuvante strategie vereist.

unit (TRU) subtype and TP53 wild-type
without clinicopathologic risk factors might not need adjuvant EFGR-TKIs. In
the high-risk group, with non-TRU subtype and/or TP53-mutation, a novel adjuvant strategy of combination of EGFR-TKI
and chemotherapy or antiangiogenic agents needs to be investigated. In patients
with APOBEC mutation signature, an alternative adjuvant strategy might be
needed.
