De studie includeerde 604 patiënten die complete resectie ondergingen voor stadium I tot en met III LUAD. Next-generation sequencing identificeerde 374 patiënten met KRAS-wildtype tumoren (62%), 95 patiënten met KRAS-G12C gemuteerde tumoren (16%), en 135 patiënten met tumoren met andere KRAS-mutatie (22%). De drie-jaars ziektevrije overleving was niet verschillend tussen de groepen met wildtype en andere mutaties, maar was significant slechter in de groep met G12C-mutatie dan in de groep met andere mutaties. Deze bevinding werd extern gevalideerd in een onafhankelijk cohort in The Cancer Genome Atlas. De groep met G12C-mutatie vergeleken met andere mutatie had meer lymfovasculaire invasie (51% versus 37%; p=0,032) en hogere tumormutatiebelasting (7,0 versus 6,1 mut/Mb; p=0,021).
De onderzoekers concluderen dat KRAS-G12C vergeleken met andere KRAS-tumoren geassocieerd is met verhoogd risico van recidief na complete resectie voor stadium I tot en met III LUAD, samenhangend met meer agressieve-ziektekenmerken. Patiënten met deze tumoren hebben wellicht baat bij behandeling met KRASG12C-remmers.

Summary: A study at Memorial Sloan Kettering Cancer Center (New York) found that KRAS-G12C mutations, compared with KRAS-wt or KRAS-other mutations, were associated with worse DFS after complete resection of stage I-III lung adenocarcinoma. The authors suggest that KRASG12C inhitors may be investigated to improve survival in this subgroup.
