Er is
veelbelovende activiteit gezien van verlengde infusie van lage-dosering
gemcitabine (PLDG) in combinatie met platina-chemotherapie voor (met name
squameus) niet-kleincellig longcarcinoom. Een fase-3 non-inferioriteitsstudie
in Tata Memorial Hospital (Mumbai, India) heeft PLDG plus carboplatine voor
squameus NSCLC vergeleken met standaard-dosering gemcitabine (STDG) plus
carboplatine voor squameus NSCLC. Prof. Kumar Prabhash en collega’s publiceren
de studie in The Lancet EClinicalMedicine.1
De studie
includeerde volwassen patiënten met stadium IIIB of IV squameus NSCLC en ECOG
performance status 2 of beter. Ze werden gerandomiseerd naar ten hoogste zes
cycli carboplatine plus PLDG (intraveneus 250 mg/m2 over zes uur,
dagen één en acht, n=153) of carboplatine plus STDG (intraveneus 1000 mg/m2
over dertig minuten, dagen één en acht, n=155). De tumorrespons werd tijdens de
behandeling iedere twee cycli beoordeeld, en vervolgens iedere twee maanden tot
progressie. Het primaire eindpunt was overall
survival. De mediane OS was 8,4 maanden in de PLDG-arm versus 6,8 maanden
in de STDG-arm (HR 0,890; 95%-bti
0,725-1,092). Er waren evenmin statistisch significante verschillen tussen
beide armen voor progressievrije overleving of optreden van adverse events.
De
onderzoekers concluderen dat de studie suggereert dat carboplatine plus PLDG
een verantwoord alternatief is voor carboplatine plus STDG.

months in the low dose arm versus 6.8 months in the standard dose arm. There
were no statistically significant differences in PFS and adverse event rates
between the arms.
