De Chinese
multicenter fase 3-studie CameL randomiseerde patiënten met gevorderd
niet-squameus niet-kleincellig longcarcinoom (ansqNSCLC) naar camrelizumab plus
carboplatine en pemetrexed of alleen carboplatine en pemetrexed. In 2021 is
gepubliceerd dat de mediane progressievrije overleving significant langer was
in de camrelizumabgroep. Prof. Caicun Zhou (Tongji Universiteit, Shanghai) en
collega’s publiceren in het Journal of Thoracic
Oncology lange-termijn resultaten van de studie.1
De patiënten
werden 1:1 gerandomiseerd naar vier tot zes cycli eerstelijns chemotherapie met
(n=205) of zonder (n=207) camrelizumab, gevolgd door onderhoudsbehandeling met
camrelizumab plus pemetrexed of alleen pemetrexed. De nu gepubliceerde analyse
werd uitgevoerd 43,9 maanden na randomisatie van de laatste patiënt. De mediane
overall survival was 27,1 maanden in de groep met camrelizumab versus 19,8
maanden in de groep zonder camrelizumab (HR 0,72; 95%-bti 0,57-0,92). Na
correctie voor crossover na progressie in de alleen-chemotherapie arm naar
camrelizumab monotherapie (n=95; 45,9%) was het overlevingsprofijt met
camrelizumab nog meer geprononceerd (HR 0,33; 95%-bti 0,42-0,71). Er waren geen
nieuwe veiligheidssignalen.
De
onderzoekers concluderen dat toevoegen van camrelizumab aan eerstelijns
chemotherapie voor ansqNSCLC resulteerde in significante verlenging van de overall survival, met manageable toxiciteit.

The multicenter phase 3 CameL trial in China
evaluated addition of camrelizumab to first-line carboplatin and pemetrexed
chemotherapy for advanced non-squamous NSCLC. Long-term follow-up of the trial
(minimum 43.9 months after randomization of the last patient) found median
overall survival 27.1 months in the camrelizumab-chemotherapy group versus 19.8
months in the chemotherapy group (HR 0.72; 95% CI 0.57-0.92).
