De
chirurgische behandelingen voor vroeg-stadium mammacarcinoom (EBC) zijn
borstsparende chirurgie met gehele-borst radiotherapie (BCS-WBRT) en totale
mastectomie (TM) zonder WBRT. Een retrospectieve studie in Taiwan heeft de
incidentie van stemmingsstoornis (mood disorder; MD) in de tien jaar na de
chirurgie geïnventariseerd. Dr. Dai-Wei Liu (Tzu Chi Ziekenhuis, Hualien) en

In de National Health Insurance Reseach Database
over de periode van begin 2000 tot eind 2013 identificeerden de onderzoekers 876
EBC-patiënten die BCS-WBRT ondergingen en 1949 patiënten die TM ondergingen. Na
propensity score matching bestonden beide groepen uit 876 patiënten. De
tien-jaars incidentie van MD was lager in de BCS-WBRT groep dan in de TM-groep:
in multivariate analyse aHR 0,69; p<0,001). Vooral depressie was minder
frequent in de BCS-WBRT groep dan in de TM-groep (p=0,004).
De onderzoekers
concluderen dat onder EBC-patiënten BCS-WBRT geassocieerd was met een lagere
tien-jaars incidentie van MD dan TM zonder WBRT.
