Dr. Eric
Horwitz (Fox Chase Cancer Center, Philadelphia PA) en collega’s hebben een
gerandomiseerde fase 3-studie uitgevoerd van conventionele (CIMRT) versus
hypogefractioneerde (HIMRT) radiotherapie voor gelokaliseerd prostaatcarcinoom.
Ze publiceren online in het International
Journal of Radiation Oncology lange-termijn patiënt-gerapporteerde uitkomsten van de studie.1 Tussen 2002 en
2007 randomiseerde de studie 303 patiënten naar 76 Gy in 38 fracties versus
70,2 Gy in 26 fracties. De kwaliteit van leven werd bepaald met de Expanded Prostate Cancer Index Composite
(EPIC), International Prostate Symptom
Score (IPSS), en de EuroQoL
(EQ5D) vragenlijsten.
Baseline EPIC, IPSS, of EQ5D gegevens
waren beschikbaar voor respectievelijk 286, 299, en 218 patiënten. Deze
patiënten werden geïncludeerd in de analyses. Overall was er geen statistisch
significant verschil tussen beide groepen in termen van verloop in de tijd van
EPIC, IPSS, of EQ5D scores, hoewel er een trend was van lagere
EPIC-incontinentie scores in de HIMRT-arm. In multivariate analyse was er geen
associatie tussen radiotherapie-fractieschema en enige QoL-parameter.
De
onderzoekers concluderen dat de studie bijdraagt aan de toenemende aanwijzingen
dat HIMRT een acceptabele behandeloptie is voor gelokaliseerd prostaatcarcinoom
