De fase
2-studie OPRA (‘organ preservation in
rectal adenocarcinoma’) randomiseerde patiënten met stadium II of III
rectumcarcinoom naar inductie-chemotherapie gevolgd door chemoradiotherapie
(INCT-CRT; n=158) of chemoradiotherapie gevolgd door consolidatie-chemotherapie
(CRT-CNCT; n=166). Patiënten met complete of bijna-complete respons na deze
therapie kregen watch and wait (WW)
aangeboden, terwijl patiënten met incomplete respons totale mesorectale excisie
(TME) aangeraden werd. Het primaire eindpunt was ziektevrije overleving, en het
secundaire eindpunt was TME-vrije overleving. Dr. Julio Garcia-Aguilar
(Memorial Sloan Kettering Cancer Center, New York) en collega’s publiceren lange-termijn resultaten van de studie in het Journal of Clinical Oncology.1
Op het
moment van de nu gepubliceerde analyse was de mediane follow-up 5,1 jaar. De
vijf-jaars percentages waren 71% voor de INCT-CRT groep en 69% voor de CRT-CNCT
groep voor DFS (p=0,68) 39% en 54% voor TME-vrije overleving (p=0,012). Er
waren 81 patiënten met hergroei; 94% in de eerste twee jaar en 99% in de eerste
drie jaar. De DFS-percentages waren niet verschillend tussen de groep patiënten
die TME kregen na restaging en de patiënten in de WW-groep die alsnog TME
kregen (64% in beide groepen).
De
onderzoekers concluderen dat deze lange-termijn resultaten laten zien dat na
totale neoadjuvante therapie voor stadium II of III rectumcarcinoom
orgaanpreservatie mogelijk is in ongeveer de helft van de patiënten, en dat
onder WW-patiënten de meeste gevallen van hergroei plaatsvinden in de eerste
twee jaar.

Summary: Long-term follow-up of the phase 2 OPRA
study found that among patients who receive total neoadjuvant therapy for stage
II or III rectal cancer, organ preservation was achieved in half of the
patients. Among patients who enter watch-and-wait after therapy, most cases of
tumor regrowth occur in the first two years.
