De
gerandomiseerde fase 2-studie CAO/ARO/AIO-12 in achttien centra in Duitsland
zocht naar de optimale volgorde van chemoradiotherapie (CRT) en chemotherapie
(CT) in de neoadjuvante behandeling van lokaal-gevorderd rectumcarcinoom (LARC),
voorafgaand aan totale mesorectale excisie (TME). In 2019 is gepubliceerd dat upfront CRT vergeleken met upfront CT resulteerde in betere
compliantie met CRT maar slechtere compliantie met CT, en in hoger percentage
patiënten met pathologisch complete respons. Prof. Emmanouil Fokas
(Goethe-Universität, Frankfurt am Main) en collega’s publiceren nu in JAMA Oncology lange-termijn
resultaten van de studie.1
De studie
includeerde 306 evalueerbare patiënten met cT3-4 en/of klierpositief
rectum-adenocarcinoom, die werden gerandomiseerd naar groep A (n=156; drie
cycli fluorouracil, leucovorine, en oxaliplatine gevolgd door
fluorouracil/oxaliplatine CRT 50,4 Gy) of groep B (n=150; CRT voorafgaand aan
CT). TME was gepland op dag 123 na start van de neoadjuvante behandeling. Na
mediaan 43 maanden follow-up (range 35-60) was in beide groepen 73% van de
patiënten ziektevrij (p=0,82). Er waren ook geen significante verschillen voor
de drie-jaars cumulatieve incidentie van locoregionaal recidief (p=0,67) of
afstandsmetastase (p=0,52). Chronische graad 3 of 4 toxiciteit na drie jaar
werd gezien in 11,8% in groep A versus 9,9% in groep B.
Ontlastings-incontinentie werd in beide groepen een jaar na randomizatie
slechter, en verbeterde licht na drie jaar.
De
onderzoekers concluderen dat CRT gevolgd door CT, vergeleken met CT gevolgd
door CRT, resulteerde in hoger pCR zonder lange-termijn oncologische of QOL
eindpunten te compromitteren. CRT gevolgd door CR dient te worden gezien als de
geprefereerde sequentie, indien orgaanpreservatie een prioriteit is.

Summary: Secondary analysis of the
multicenter randomized phase 2 CAO/ARO/AIO-12 study in Germany showed that
among patients with locally advanced rectal cancer, neoadjuvant
chemoradiotherapy followed by chemotherapy compared with CT followed by CRT
resulted in higher pathological complete response without compromising
disease-free survival, toxicity, quality of life, or stool incontinence.
