Er is weinig
informatie over uitkomsten van base of skull (BOS)-chordomen in de pediatrische
populatie. Een retrospectieve studie van Massachusetts General Hospital
(Boston) heeft lange-termijn uitkomsten met chirurgie en protonenradiotherapie
(PRT) voor BOS-chordomen geïnventariseerd. Dr. Shannon MacDonald en collega’s
publiceren de studie in Neuro-Oncology.1
De studie
includeerde 204 patiënten die tussen 1981 en 2021 in MGH na chirurgie PRT of
combinatie van PRT en fotonradiotherapie kregen. De mediane leeftjd bij
diagnose was 11,1 jaar (range 1-21). Vijftien patiënten (7%) kregen
chemotherapie voorafgaand aan de radiotherapie. De mediane dosering was 76,7 Gy
(RBE). Na mediaan 10 jaar follow-up (IQR 5-16) was recidief gezien in 56
patiënten (28%). De mediane PFS was 25 jaar en de mediane OS was 26 jaar. De
radiotherapie werd goed verdragen, met milde of matige toxitieiten in 80% en
secundaire stralings-gerelateerde maligniteiten in vier patiënten (2%).
De
onderzoekers concluderen dat onder pediatrische patiënten met BOS-chordomen
hoge-dosering PRT na chirurgie een effectieve benadering is met lage
toxiciteit.
