De fase
2-studie CheckMate 204, in 28 centra in de Verenigde Staten, includeerde
patiënten met niet-bestraalde hersenmetastasen van melanoom (MBM). De patiënten
kregen de combinatie nivolumab (1 mg/kg) plus ipilimumab (3 mg/kg) eens per
drie weken voor vier doses gevolgd door nivolumab 3 mg/kg iedere twee weken. In
2018 is gepubliceerd
gepubliceerd dat de combinatie werkzaam was in patiënten met asymptomatische
MBMs. In patiënten met symptomatische MBMs was de werkzaamheid beperkt. Prof. Hussein Tawbi (MD Anderson Cancer Center,
Houston TX) en collega’s publiceren in The Lancet Oncology lange-termijn
resultaten van de studie.1
De studie
includeerde 101 patiënten met asymptomatische MBMs (cohort A; ECOG performance
status 0 of 1) en 18 patiënten met symptomatische MBMs (cohort B; ECOG PS 2 of
lager). De mediane follow-up was 34,4 maanden (IQR 14,7-36,4) in cohort A en
7,5 maanden (1,2-35,2) in cohort B. Intracranieel klinisch profijt, beoordeeld
door lokale onderzoekers, werd gezien in 57,4% (95%-bti 47,2-67,2) van de
patiënten in cohort A en 16,7% (3,6-41,4) in cohort B. Complete respons werd
gezien in 53,5% respectievelijk 16,7%. Het intracraniële-progressievrije
overlevings-percentage na drie jaar was 54,1% versus 18,9%, en het drie-jaars overall survival percentage was 71,9%
versus 36,6%. De meest-gerapporteerde treatment-related
adverse events waren colitis, diarree, hypofysitis, en verhoogd
alanine-aminotransferase (ieder in 5% van de patiënten) in cohort A; er waren
geen TRAEs in meer dan één patiënt in cohort B. Er was één graad 5 TRAE in
cohort A (myocarditis).
De
onderzoekers concluderen dat de combinatie van nivolumab en ipilimumab duurzame
werkzaamheid had voor patiënten met asymptomatische MBMs. Symptomatische ziekte
in patiënten met MBMs blijft moeilijk te behandelen, maar sommige patiënten
hadden lange-termijn profijt van de combinatie.

Summary: Final results of the multicenter
phase 2 CheckMate 204 study show that the combination of nivolumab plus
ipilimumab resulted in durable 3-year response, progression-free survival, and
overall survival rates among patients with asymptomatic melanoma brain
metastases (MBMs). Symptomatic disease in patients with MBMs remains difficult
to treat, but some patients achieved a long-term response with the combination.
