Cryotherapie
en brachytherapie zijn definitieve lokale behandelopties voor laag- en intermediair-risico
prostaatcarcinoom. Er is uitgebreide evidentie voor de werkzaamheid van
brachytherapie, maar de aanwijzingen voor werkzaamheid van cryotherapie
berusten voornamelijk op single-institution
retrospectieve studies, schrijven dr. Gregory Swanson (Texas A&M University,
Temple) en collega’s in een publicatie die online is in het International
Journal of Radiation Oncology.1 In de publicatie presenteren ze
uitkomsten van een directe vergelijking van low
dose rate (LDR) brachytherapie en cryotherapie voor laag- en
intermediair-risico prostaatcarcinoom.
Tussen 1990
en 2013 kregen in het ziekenhuis van Texas A&M University 50 patiënten
cryotherapie voor laag-risico prostaatcarcinoom (LRC-groep), 92 patiënten
cryotherapie voor intermediair-risico prostaatcarcinoom (IRC-groep), 133
patiënten brachytherapie voor laag-risico prostaatcarcinoom (LRB), en 84
patiënten brachytherapie voor intermediair-risico prostaatcarcinoom (IRB). De
mediane PSA follow-up was 85,6 maanden voor de LRC-groep; 59,2 maanden voor de
IRC-groep; 74,9 maanden voor de LRB-groep, en 59,8 maanden voor de IRB-groep.
De vijf-jaars biochemisch-progressievrije overleving (bPFS) was 57,9% in de
cryotherapiegroepen versus 89,6% in de brachytherapiegroepen (p<0,0001).
Uitgesplitst naar risicogroepen was de vijf-jaars bPFS 70,0% voor LRC; 51,4%
voor IRC; 89,4% voor LRB; en 89,7% voor IRB. Voor zowel de laag- als de intermediair-risicogroepen
was bPFS significant (p<0,05) verschillend tussen LDR-brachytherapie en cryotherapie.
De
onderzoekers concluderen dat de studie suggereert dat cryotherapie voor laag-
en intermediair-risico prostaatcarcinoom inferieur is aan LDR-brachytherapie.
