Chirurgische
resectie resulteert in overlevingsprofijt onder patiënten met diffuus
laaggradig glioom (DLGG), maar de associatie van resectie met functionele
uitkomsten is niet duidelijk. Een systematisch overzicht en meta-analyse van
gepubliceerde studies heeft de associatie tussen extent of resection (EoR) en functionele uitkomsten in volwassen
DLGG-patiënten onderzocht. Dr. Michael Tin Chung Poon (University of Edinburgh,
UK) en collega’s publiceren de analyse in het Journal of Neuro-Oncology.1
In Medline,
Embase, en Central tot 19 februari 2021 vonden de onderzoekers zeven voor het
onderwerp relevante observationele studies met matige-tot-hoge kwaliteit
(Newcastle-Ottawa Scale hoger dan 6). De zeven studies telden tezamen 234
patiënten met DLGG. Twee studies rapporteerden neurocognitie, drie performance
status, één kwaliteit van leven, en zes return
to work (RTW). In meta-analyse was het RTW-percentage binnen twaalf maanden
na resectie 84% (95%-bti 50-96; I2 38%) na bruto-totale resectie,
66% (95%-bti 14-96; I2
57%) na subtotale resectie, en 31% (95%-bti 4-82; I2 0%) na partiële resectie. Er waren onvoldoende data
over andere functionele uitkomsten voor kwantitatieve synthese.
De onderzoekers
concluderen dat een hoger percentage DLGG-patiënten weer aan het werk ging na
GTR dan na STR of PR.

Summary: Meta-analysis of six moderate to
high quality observational studies found that among patients with diffuse
low-grade glioma higher proportion returned to work following gross total
resection compared with those who had a subtotal or partial resection. There
were insufficient data on other functional outcomes for quantitative synthesis.
