Er zijn aanwijzingen dat microsatellite instalibity high status (MSI-h) van resectabel maagcarcinoom (GC) een gunstige prognostische factor en misschien ook een negatieve predictieve factor voor neoadjuvante of adjuvante chemotherapie is. Een individual patient data meta-analyse heeft de waarde van deze aanwijzingen onderzocht. Dr. Elizabeth Smyth (Cambridge University UK) en collega’s publiceren de meta-analyse online in het Journal of Clinical Oncology.¹

De meta-analyse includeerde vier studies (MAGIC, CLASSIC, ARTIST, en ITACA-S) die tezamen 1556 patiënten met bekende MSI-status telden. Er waren 121 patiënten met MSI-h status (7,8%). De vijf-jaars ziektevrije overleving was 71,8% in de MSI-h groep versus 52,3% in de MSI-low/microsatellite stable (MSI-l, MSS) groep; de vijf jaars overall survival was 77,5% versus 59,3% in de twee groepen. In multivariate analyse was MSI geassocieerd met langere DFS (HR 1,88; p<0,001) en OS (HR 1,78; p=0,008). Patiënten met MSI-low/MSS GC hadden profijt van toevoeging van chemotherapie aan chirurgie (vijf-jaars DFS 57% en vijf-jaars OS 62% met chirurgie plus chemotherapie versus 41% en 53% met alleen chirurgie). Patiënten met MSI-high GC hadden geen profijt van toevoeging van chemotherapie aan chirurgie (vijf-jaars DFS 70% en vijf-jaars OS 75% met chirurgie plus chemotherapie versus 77% en 83% met alleen chirurgie).

De onderzoekers concluderen dat in patiënten met resectabel primair GS, de MSI-status een robuuste prognostische en chemotherapie-predictieve marker was.
