In de literatuur tussen begin 2000 en begin 2021 identificeerden de onderzoekers 128 studies met tezamen 47.432 patiënten die standaard-behandelingen hadden uitgeput en werden gerandomiseerd naar een oncologiemedicatie of beste ondersteunende zorg al of niet met placebo. De HRs voor PFS en OS waren 0,58 (95%-bti 0,53-0,63) en 0,82 (95%-bti 0,78-0,85). Het absolute profijt was echter beperkt met PFS- en OS-winsten van 2,1 en 0,5 maanden. De absolute excessen in all grade, graad 3 en 4, en graad 5 adverse events waren +13,9%; +10,2%; en +0,5%. De correlatie tussen excess ernstige toxiciteit en werkzaamheid was zwak (R2 < 0,2).
De onderzoekers concluderen dat patiënten profijt hadden bij anticancer drugs geëvalueerd in RCTs zonder actieve comparator. Ernstige toxiciteit was niet significant geassocieerd met werkzaamheid.

Summary: A systematic review and meta-analysis of 128 trials involving 47,432 advanced cancer patients who had exhausted standard of care options found that the pooled HRs for PFS and OS were 0.58 (95% CI 0.53-0.63) and 0.82 (95% CI 0.78-0.85). The absolute benefits however were limited wit PFS and OS gains of 2.1 and 0.5 months. The absolute excesses in all grade, grade III-IV, and grade V adverse events between the two arms were +13.9%, +10.2%, and +0.5%. The correlation between the excess of severe toxicity and efficacy was weak (all R2<0.2).
