De optimale
inductietherapie voor hoog-risico neuroblastoom is niet bekend. Het standaard-regime van de International Society of Pediatric Oncology
Europe Neuroblastoma Group is rCOJEC (rapid cisplatine, vincristine,
carboplatine, etoposide, en cyclofosfamide). Met het MSKCC-N5 regime zijn hoge responspercentages bereikt. De
multinationale gerandomiseerde HR-NBL 1.5 studie heeft beide regimes head-to-head vergeleken. Prof. Ruth
Lydia Ladenstein (Medische Universiteit Wenen) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde patienten in de leeftijd van 1 tot en met 20 jaar met stadium 4
neuroblastoom, of jonger dan 1 jaar met stadium 4/4s neuroblastoom met MYCN-amplificatie. De patiënten werden
gestratificeerd voor land en metastaselocatie gerandomiseerd naar inductie met
rCOJEC of MSKCC-N5. Na inductie volgde resectie van de primaire tumor,
hoge-dosering busulfan en melfalan, radiotherapie naar de primaire tumorlocatie,
en isotretinoïne met dinutuximab beta antilichaam met of zonder interleukine-2.
De primaire eindpunten waren metastatische complete respons en drie-jaars gebeurtenisvrije
overleving.
De studie
includeerde 630 patiënten, met een mediane leeftijd bij diagnose 3,2 jaar
(range 1 tot en met 20 jaar; zestien jonger dan 1 jaar met MYCN-amplificatie. De rCOJEC-groep telde 313 patiënten en de
MSKCC-N5 groep 317. Er waren geen significante verschillen tussen de groepen
voor het eindpunt complete respons van metastasen (32% in de rCOJEC-groep versus
35% in de MSKCC-N5 groep; p=0,368) en evenmin voor het eindpunt drie-jaars
EFS-percentage (44% versus 47%; p=0,527). De drie-jaars overall survival
percentages waren 60% versus 65% (p=0,379). In beide groepen overleed 1% aan behandelings-gerelateerde
toxiciteit. Niet-hematologische graad 3 en 4 toxiciteiten waren meer frequent
met MSKCC-N5 dan met rCOJEC (68% versus 48%; p<0,001), met name infecties,
stomatitis, misselijkheid en braken, en diarree.
De
onderzoekers concluderen dat er geen significante verschillen in werkzaamheid
waren tussen beide inductieregimes, maar dat rCOJEC resulteerde in minder acute
toxiciteit. Daarom dient rCOJEC te worden gezien als het inductieregime van
eerste keus.

Summary: The
multinational randomized HR-NBL 1.5 study compared rCOJEC versus MSKCC-N5
induction therapy regimens for high-risk neuroblastoma. There were no significant
differences between the two groups for the endpoints metastatic complete
response, 3-year event-free survival, and 3-year overall survival. Grade 3 and
4 acute toxicity was more frequent was MSKCC-N5 than with rCOJEC (68% of
patients versus 48%; p<0.001). The authors conclude that rCOJEC should be
the preferred induction regimen for high-risk neuroblastoma.
