Neoadjuvante
chemoradiotherapie (CRT) gevolgd door chirurgische resectie is
standaard-behandeling voor lokaal-gevorderd rectumcarcinoom (LARC). Een fase
multicenter 1-2 studie in Japan heeft neoadjuvante CRT gevolgd door
consolidatie met preoperatief nivolumab
en chirurgie voor LARC geëvalueerd. Dr. Takayuki Yoshino (National Cancer Center East, Chiba) en collega’s
publiceren de studie in Clinical Cancer Research.1
In fase 1
ondergingen drie patiënten CRT gevolgd door vijf cycli nivolumab zonder
noodzaak van doseringsmodificaties. Dit schema werd gekozen voor fase 2, waarin
37 patiënten met microsatellite stable (MSS) en 5 patiënten met microsatellite
instability-high (MSI-H) LARC werden geïncludeerd. CRT gevolgd door vijf cycli
nivolumab resulteerde in centraal-bevestigde pathologisch complete respons in
11 van 37 MSS-patiënten (pCR 30%; 90%-bti 18-44) en 3 van 5 MSI-H patiënten
(60%). Onder de MSS patiënten met PD-L1 tumor
proportion score 1% of hoger was het pCR-percentage 75%; onder de MSS
patiënten met PD-L1 TPS lager dan 1% was het pCR-percentage 17% (p=0,004).
De
onderzoekers concluderen dat preoperatieve CRT gevolgd door nivolumab-consolidatie
resulteerde in bemoedigende pCR-percentages.
