Er is
behoefte aan nieuwe eerstelijns behandelingen voor lokaal-gevorderd of metastatisch slokdarm squameus celcarcinoom
(ESCC) en maag/maagslokdarmovergang (G/GEJ) adenocarcinoom. Een multicenter
fase 2-studie in China heeft de combinatie van het anti-PD-1 monoklonaal
antilichaam tislelizumab plus chemotherapie voor deze maligniteiten
geëvalueerd. Dr. Jianming Xu (Vijfde Medisch Centrum van het Algemene
Ziekenhuis van het Chinese Volksbevrijdingsleger, Beijing) en collega’s
publiceren de studie in Clinical Cancer Research.1
De studie
includeerde 15 patiënten met gevorderd ESCC en 15 patiënten met gevorderd
G/GEJ-adenocarcinoom. De ESCC-patiënten kregen intraveneus tislelizumab 200 mg
iedere drie weken plus ten hoogste zes cycli cisplatine en fluorouracil. De
G/GEJ-adenocarcinoom patiënten kregen tislelizumab plus ten hoogste zes cycli
oxaliplatine en capecitabine. Het primaire eindpunt van de studie was
veiligheid. Eén patiënt overleed aan als met de behandeling samenhangend
beoordeelde leverdysfunctie. De meest-gerapporteerde adverse events waren anemie (n=18), verminderde eetlust (n=17),
misselijkheid (n=16), en asthenie (n=15). In beide cohorten werd objectieve
respons gezien in 46,7% en ziektecontrole in 80%. De mediane duur van respons
was 12,8 maanden in het ESCC-cohort en was nog niet matuur in het
G/GEJ-adenocarcinoom cohort.
De
onderzoekers concluderen dat eerstelijns tislelizumab plus chemotherapie voor
gevorderd ESCC of G/GEJ adenocarcinoom resulteerde in duurzame responsen met manageable toxiciteit.
