De klinische
uitkomsten van volwassenen met AML zijn met de huidige behandelingen niet
bevredigend. Een fase 2-studie in drie centra in China heeft de eerstelijns combinatie
van de Bcl-2 remmer venetoclax met 3+ 7 daunorubicine en cytarabine
chemotherapie (DAV) voor AML in
volwassenen geëvalueerd. Prof. Hong-Hu Zhu (Universiteit van Zhejiang) en
collega’s publiceren de studie in The Lancet Haematology.1
De studie
includeerde patiënten in de leeftijd van achttien tot en met zestig jaar, met
ECOG performance status 2 of beter, en niet-eerder behandeld AML. De patiënten
kregen één of twee inductiecycli DAV. Het primaire eindpunt was percentage
patiënten met complete remissie (CR of CRi) na de eerste cyclus. Complete
remissie na de eerste cyclus werd gezien in 30 van 33 patiënten (91,%; 95%-bti
76-98), onder wie 29 zonder detecteerbare residuele ziekte (<0,1%). Graad 3
of 4 neutropenie, trombocytopenie, en anemie werden gezien in alle patiënten;
febriel neutropenie in 55%, pneumonie in 21%, en sepsis in 12%. Er waren geen
graad 5 toxiciteiten. De mediane follow-up was 11 maanden (IQR 9-12). Het één-jaars
gebeurtenisvrije overlevingspercentage was 72% (95%-bti 56-94) en het één-jaars
overall survival percentage was 97% (91-100).
De
onderzoekers concluderen dat het DAV-regime een effectieve inductietherapie was
voor nieuw-gediagnostiseerd AML in volwassen patiënten.

induction therapy for newly diagnosed AML in adult patients.
