De mainstay van behandeling voor vroeg-stadium folliculair lymfoom (FL) is locale radiotherapie. De fase 2-studie FIL MIRO, in 27 centra in Italië, evalueerde toevoeging van het anti-CD20 monoklonale antilichaam ofatumumab aan de behandeling in patiënten die na radiotherapie minimaal residuele ziekte (MRD)-positief waren. Prof. Alessandro Pulsoni (S.M. Goretti Ziekenhuis, Latina) en collega’s publiceren de studie in The Lancet Haematology.¹

De studie includeerde volwassen patiënten met nieuw-gediagnostiseerd stadium I of II FL. De patiënten kregen 24 Gy involved-field radiotherapie over 12 dagen. Patiënten die na de radiotherapie of tijdens de follow-up MRD-positief waren kregen acht intraveneuze doses ofatumumab 1000 mg eens per week. Het primaire eindpunt was het percentage patiënten met BCL2::IGH-rearrangement in perifeer bloed of beenmerg die na de radiotherapie MRD-positief waren en MRD-negatief werden na de ofatumumab-behandeling.

De studie includeerde 106 patiënten (53% vrouwen) die radiotherapie kregen. Onder de 105 patiënten die evalueerbaar waren voor BCL2::IGH status hadden 32 (30%) voor aanvang van de behandeling een detecteerbaar rearrangement. Na de radiotherapie hadden 12 van 30 patiënten (40%) MRD-negatieve status bereikt, die tenminste 36 maanden aanhield in drie patiënten (25%). Onder de patiënten die na de radiotherapie MRD-positief waren resulteerde ofatumumab-behandeling in MRD-negativiteit in 23 van 25 patiënten (92%). Na mediaan 46,1 maanden follow-up waren 14 van deze patiënten (61%) nog steeds MRD-negatief. De meest-waargenomen graad 3 of 4 adverse events waren infusiereacties, in 4 patiënten.

De onderzoekers concluderen dat locale radiotherapie vaak niet voldoende is voor de eradicatie van FL. Een MRD-geleide consolidatie met ofatumumab leidde tot moleculaire remissie in bijna alle patiënten.

Summary: The multicenter phase 2 FIL MIRO trial in Italy found that among patients with early-stage follicular lymphoma, local radiotherapy is frequently not associated with the eradication of follicular lymphoma. MRD-guided, anti-CD20 monoclonal antibody consolidation enabled molecular remission in almost all patients.
