De
standaard-behandeling voor locoregionaal-gevorderd nasofarynxcarcinoom (NPC) is
platina-gebaseerde concurrente chemoradiotherapie (CRT). Een fase 3-studie in
China en Singapore heeft de waarde onderzocht van toevoegen van
gemcitabine-cisplatine inductiechemotherapie aan deze behandeling. Prof. Jun Ma
(Sun Yat-sen Universiteit, Guangzhou) en collega’s publiceren de studie online in The New England Journal
of Medicine.1
De studie
includeerde 480 patiënten met locoregionaal-gevorderd NPC. Ze werden 1:1 gerandomiseerd
naar drie cycli gemcitabine-cisplatine inductiechemotherapie gevolgd door CRT
met concurrent cisplatine en IMRT (inductiechemotherapiegroep; n=242) of alleen
CRT (standaardtherapiegroep; n=238). Het primaire eindpunt van de studie was
recidiefvrije overleving. Secundaire eindpunten waren overall survival, veiligheid, en adherentie aan de behandeling.
De mediane
follow-up was 42,7 maanden. De drie-jaars RFS was 85,3% in de inductiechemotherapiegroep versus 76,5 % in de
standaardtherapiegroep (HR 0,51; p=0,001). De drie-jaars OS was 94,6% versus
90,3% (HR 0,43; 95%-bti 0,24-0,77). De incidentie van graad 3 of 4 acute adverse events was 75,7% versus 55,7%;
en de incidentie van graad 3 of 4 late toxische effecten was 9,2% versus 11,4%.
Drie cycli inductiechemotherapie werd voltooid door 96,7% van de patiënten in
de inductiechemotherapiegroep.
De
onderzoekers concluderen dat toevoegen van gemcitabine-cisplatine
inductiechemotherapie aan de standaard-behandeling resulteerde in significante
verbetering van RFS en OS in patiënten met lokaal-gevorderd NPC.
