Inductiechemotherapie
met docetaxel en cisplatine plus fluoruracil (TPF) is standaard voor
locoregionaal-gevorderd nasofarynxcarcinoom (LA-NPC) maar is geassocieerd met
aanzienlijke bijwerkingen, zoals myelosuppressie en diarree. Een multicenter noninferioriteits
fase 3-studie in China heeft onderzocht of fluorouracil veilig uit dit regime
weggelaten kan worden. Prof. Yong Chen (Sun Yat-sen Universiteit, Guangzhou) en
collega’s publiceren de studie in eClinicalMedicine.1
De studie
includeerde 361 patiënten met stadium III-IVA (behalve T3-4N0) NPC van zes
centra in China. De patiënten werden gerandomiseerd naar drie drie-weekse cycli
TP (n=181) of TPF (n=180) inductiechemotherapie, gevolgd door concurrente
chemoradiotherapie. Het primaire eindpunt was twee-jaars
faalvrije-overlevingspercentage, met een noninferioriteitsmarge van -10% voor
TP versus TPF. De figuur laat zien dat de TP-groep significant betere twee-jaars FFS had dan
de TPF-groep, en eveneens een trend van betere overall survival. De studie werd voortijdig gestopt. Tijdens die
inductiechemotherapie hadden de patiënten in de TPF-groep vergeleken met
patiënten in de TP-groep hogere frequentie van graad 1 of 2 neutropenie (30,0%
versus 15,7%; p=0,0010), graad 1 of 2 diarree (11,3% versus 5,1%; p=0,032), en
graad 3 of 4 neutropenie (24,3% versus 14,0%; p=0,014).
De
onderzoekers concluderen dat TP-inductiechemotherapie voor LA-NPC in termenvan
faalvrije overleving duidelijk niet-inferieur was aan
TPF-inductiechemotherapie, en beter verdragen werd.

better tolerated.
