De standaard
eerstelijns behandeling voor gevorderd of metastatisch squameus celcarcinoom
van de slokdarm (A/M ESCC) is chemotherapie, hoewel de prognose slecht blijft.
De gerandomiseerde fase 3-studie ESCORT-1st in zestig ziekenhuizen in China
heeft toevoegen van camrelizumab (anti-PD-1) aan eerstelijns chemotherapie voor
A/M ESCC geëvalueerd. Prof. Rui-Hua Xu (Sun Yat-sen Universiteit, Guangzhou) en
collega’s publiceren de studie in JAMA.1
De studie
includeerde 596 patiënten (87,8% mannen; mediane leeftijd 62 jaar) die 1:1
werden gerandomiseerd naar intraveneus camrelizumab 200 mg (n=298) of placebo
(n=298) eens per drie weken toegevoegd aan zes cycli paclitaxel-cisplatine. De
primaire eindpunten waren overall survival en progressievrije overleving. De
mediane follow-up was 10,8 maanden. De figuur laat zien dat de mediane OS 15,3 maanden was met
camrelizumab-chemotherapie versus 12,0 maanden met placebo-chemotherapie (HR
0,70; p=0,001). De mediane progressievrije overleving was 6,9 maanden versus
5,0 maanden (HR 0,56; p<0,001). Graad 3 of hoger treatment-related adverse events werden gerapporteerd voor 63,4%
van de patiënten met camrelizumab-chemotherapie versus 67,7% van de patiënten
met placebo-chemotherapie, inclusief graad 5 TRAEs voor 3,0% versus 3,7%.
De
onderzoekers concluderen dat onder patiënten met A?M ESCC toevoeging van
camrelizumab aan eerstelijns chemotherapie resulteerde in significante
verbetering van OS en PFS.

months with camrelizumab plus chemotherapy versus 12.0 months with placebo plus
chemotherapy (HR 0.70; p=0.001). Progression-free survival was 6.9 versus 5.6
months (HR 0.56; p<0.001).
