Anaplastisch
grootcellig lymfoom (ALCL) wordt in vrijwel alle pediatrische patiënten
gekenmerkt door translocatie in het ALK-gen.
Crizotinib (CZ) is een ALK-remmer. De CZ-arm van de multicenter ANHL12P1-studie
van de Children’s Oncology Group evalueerde CZ toegevoegd aan chemotherapie
voor nieuw-gediagnostiseerd niet-gelokaliseerd ALK+ CD30+ ALCL in pediatrische
patiënten. Dr. Eric Lowe

De CZ-arm
includeerde 66 kinderen, die zes drie-weekse cycli chemotherapie kregen met CZ tweemaal daags. Het
twee-jaars gebeurtenisvrije overlevingspercentage was 76,8% (95%-bti 68,5-88,1)
en het twee-jaars overall survival
percentage was 95,2% (85,7-98,4). Een graad 2 of hoger tromboëmbolische
gebeurtenis werd gezien in dertien patiënten (19,7%; 95%-bti 11,1-31,1). Onder
de 25 patiënten die profylactische anticoagulantia kregen waren er twee met een
tromboëmbolische gebeurtenis (8,0%; 95%-bti 0,01-26). Het twee-jaars EFS-percentage
was 85,6% (68,6-93,3) onder patiënten met minimal
disseminated disease (MDD)-negatieve ziekte en 58,1% (33,4-76,4) onder
patiënten met MDD-positieve ziekte.
De
onderzoekers concluderen dat CZ toegevoegd aan standaard chemotherapie bijdroeg
aan de preventie van relapse, dat de MDD-status voorspellend was voor EFS, en
dat de behandeling resulteerde in onverwachte tromboëmbolische gebeurtenissen.

Summary: The
CZ arm of the Children’s Oncology Group trial ANHL12P1 evaluated CZ added to
chemotherapy for newly diagnosed nonlocalized ALK+ CD30+ ALCL. The event-free
survival was higher than what was seen in most previous studies and similar to
that observed in a study using brentuximab vedotin and chemotherapy. Twenty
percent of patients experienced a thromboembolic event if they were not receiving
anticoagulation.
