De fase 1b-studie COSMIC-021 evalueert de combinatie van cabozantinib plus atezolizumab voor solide tumoren. Expansiecohort 6 van de studie includeerde patiënten met metastatisch castratieresistent prostaatcarcinoom (mCRPC) met progressie op moderne hormoontherapie zoals abirateron of enzalutamide, een populatie met slechte prognose en weinig behandelopties. Dr. Neeraj Agarwal (University of Utah, Salt Lake City) en collega’s publiceren resultaten van het cohort in The Lancet Oncology.¹

Cohort 6 includeerde 132 volwassen patiënten van 42 centra in Frankrijk, Italië, Nederland, Spanje, en de Verenigde Staten. De patiënten hadden mCRPC met progressie na abirateron of enzalutamide of beide, en een ECOG performance status 0 of 1. Ze kregen oraal cabozantinib 40 mg eens per dag en intraveneus atezolizumab 1200 mg iedere drie weken. De behandeling werd voortgezet tot progressie of niet-acceptabele toxiciteit. Het primaire eindpunt was lokaal-beoordeelde objective response rate.

Op het moment van de nu gepubliceerde analyse was de mediane duur van follow-up 15,2 maanden (IQR 9,6-21,7). Complete respons werd gezien in drie patiënten en partiële respons in 28 patiënten, voor een ORR van 23% (95%-bti 17-32). Graad 3 of 4 treatment-related adverse events werden gezien in 72 patiënten (55%), ernstige adverse events of any causality in 74 patiënten (56%), en TRAEs die tot discontinuering van de behandeling leidden in 28 patiënten (21%). Eén patiënt werd getroffen door een graad 5 TRAE (dehydratie).

De onderzoekers concluderen dat de combinatie van cabozantinib en atezolizumab veelbelovende antitumorwerkzaamheid had voor mCRPC met progressie na moderne hormoontherapie. Het veiligheidsprofiel was acceptabel.

Summary: The multinational phase 1b COSMIC-021 trial evaluates the combination of cabozantinib and atezolizumab for solid tumors. Expansion cohort 6 of the trial included patients with metastatic castration-resistant prostate cancer after novel hormone therapy (eg, abiraterone or enzalutamide), a population with poor prognosis and few treatment options. The combination showed promising antitmor activity with an acceptable safety.
