Standaard
eerstelijns behandeling voor gevorderd of metastatisch squameus celcarcinoom
van de slokdarm (aESCC) is platina-gebaseerde chemotherapie. De multinationale
fase 3-studie ORIENT-15 heeft de combinatie van eerstelijns sintilimab (anti-PD-1)
en chemotherapie voor aESCC vergeleken met placebo plus chemotherapie. Prof.
Lin Shen (Peking University Cancer Hospital, Beijing) en collega’s publiceren een interimanalyse van de studie in BMJ.1
ORIENT-15
werd uitgevoerd in 66 centra in China en 13 centra buiten China (in Australië,
Frankrijk, Spanje, en de Verenigde Staten). De studie includeerde 659 volwassen
patiënten met niet-eerder systemisch behandeld aESCC. De patiënten werden 1:1
gerandomiseerd naar sintilimab (3 mg/kg in patiënten lichter dan 60 kg of flat dose 200 mg voor zwaardere
patiënten) of placebo iedere drie weken, toegevoegd aan chemotherapie (investigator’s choice uit cisplatine-paclitaxel
of cisplatine -5FU). Primaire eindpunten van de studie waren overall survival onder alle patiënten en
OS onder patiënten met PD-L1 combined
positive score ≥ 10.
De
sintilimab-chemotherapiegroep telde 327 patiënten en de placebo-chemotherapiegroep
332. De interimanalyse werd uitgevoerd na 351 OS-gebeurtenissen. De mediane
follow-up was 16,0 maanden in de sintilimab-chemotherapiegroep en 16,9 maanden
in de placebo-chemotherapiegroep. De figuur laat zien dat zowel onder alle patiënten (panel A) als onder de
patiënten met PD-L1 CPS ≥ 10 de OS significant langer was in de
sintilimab-chemotherapiegroep dan in de placebo-chemotherapiegroep. Treatment-related adverse events werden
gerapporteerd voor 98% van de patiënten in de sintilimab-chemotherapiegroep en
98% van de patiënten in de placebo-chemotherapiegroep, en graad 3 of hoger
TRAEs in 60% respectievelijk 55%.
De
onderzoekers concluderen dat eerstelijns sintilimab-chemotherapie resulteerde
in significant betere OS dan placebo-chemotherapie onder patiënten met aESCC.
