Lurbinectedine
is een selectieve remmer van oncogeentranscriptie. De multinationale fase
3-studie ATLANTIS evalueerde de combinatie van lurbinectedine en doxorubicine
als tweedelijns behandeling voor kleincellig longcarcinoom. Prof. Luis Paz-Ares
(Hospital Universitario 12 de Octubre, Madrid) en collega’s publiceren de studie in The Lancet Respiratory Medicine.1
ATLANTIS
werd uitgevoerd in 135 centra op vier continenten. De studie includeerde 613
volwassen patiënten met recidiverend SCLC na platina-gebaseerde chemotherapie.
De patiënten werden gerandomiseerd naar drie-weekse cycli van lurbinectedine-doxorubicine
(n=307) of behandelaars keus van controletherapie (n=306), bestaande uit
topotecan (n=127) of cyclofosfamide-doxorubicine-vincristine (CAV; n=179). Alle
patiënten kregen G-CSF. Het primaire eindpunt was overall survival.
De mediane
follow-up was 24,1 maanden. De mediane OS was 8,6 maanden in de lurbinectedine-doxorubicinegroep versus 7,6
maanden in de controlegroep (HR 0,97; p=0,70). Twaalf patiënten overleden aan
behandeling-gerelateerde adverse events:
twee (<1%) in de lurbinectedine-doxorubicinegroep versus tien (3%) in de
controlegroep. Graad 3 of hoger hematologische AEs waren minder frequent in de
lurbinectedine-doxorubicinegroep dan in de controlegroep: anemie 19% versus 38%,
neutropenie 37% versus 69%, en trombocytopenie 14% versus 31%. Negen procent
van de patiënten in de lurbenectedine-doxorubicinegroep versus 16% van de
patiënten in de controlegroep discontinueerde de studie vanwege AEs.
De
onderzoekers concluderen dat lurbinectedine-doxorubicine vergeleken met
controle niet resulteerde in langere OS maar wel beter verdragen werd.
