De eerstelijns combinatie van nivolumab plus ipilimumab is een werkzame behandeling voor gevorderd niet-kleincellig longcarcinoom (aNSCLC). De multinationale fase 3-studie CheckMate 9LA heeft de waarde onderzocht van nivolumab-ipilimumab plus twee cycli chemotherapie voor aNSCLC. Prof. Luis Paz-Ares (Hospital Universitario 12 de Octubre, Madrid) en collega’s publiceren de studie in The Lancet Oncology.¹

CheckMate 9LA werd uitgevoerd in 103 centra in negentien landen. De studie includeerde volwassen patiënten met behandelingsnaïef stadium IV of recidiverend NSCLC en een ECOG performance status 0 of 1. De patiënten werden gerandomiseerd naar een experimentele groep met nivolumab (intraveneus 360 mg iedere drie weken) plus ipilimumab (intraveneus 1 mg/kg iedere zes weken) plus twee drie-weekse cycli platina doublet chemotherapie; of een controlegroep met vier drie-weekse cycli chemotherapie. Het primaire eindpunt was overall survival.

De experimentele groep telde 361 patiënten en de controlegroep 358. Bij een vooraf-geplande interimanalyse, na mediaan 9,7 maanden follow-up (IQR 6,4-12,8) was de mediane OS 14,1 maanden in de experimentele groep versus 10,7 maanden in de controlegroep (HR 0,69; p=0,0005). In een exploratieve analyse met langere follow-up (mediaan 13,2 maanden; IQR 6,4-17,0) was de mediane OS 15,6 maanden in de experimentele groep versus 10,9 maanden in de controlegroep (HR 0,66; 95%-bti 0,55-0,80). De meest-gerapporteerde graad 3 of 4 adverse events waren neutropenie (7% in de experimentele groep versus 9% in de controlegroep), anemie (6% versus 14%), diarree (4% versus 1%), verhoogd lipase (6% versus 1%), en asthenie (1% versus 2%). Behandelings-gerelateerd overlijden trof zeven patiënten in de experimentele groep versus zes patiënten in de controlegroep.

De onderzoekers concluderen dat de combinatie van nivolumab plus ipilimumab en twee cycli chemotherapie resulteerde in significant betere OS vergeleken met alleen chemotherapie. De profijt/risico-verhouding van de combinatie was gunstig.

Summary: The multinational phase 3 study CheckMate 9LA compared the combination of nivolumab-ipilimumab plus two cycles of chemotherapy versus four cycles of chemotherapy alone for treatment-naïve advanced NSCLC. With median 13.2 months follow-up the median OS was 15.6 months in the combination group versus 10.9 months in the chemotherapy group (HR 0.66; 95%-bti 0.55-0.80).
