De studie includeerde 326 patiënten die venetoclax discontinueerden in de front-line (4%) of R/R-setting (96%). Het mediane aantal lijnen van behandeling voorafgaand aan venetoclax was drie; 40% van de patiënten was BTK-remmer (BTKi)-naïef, en 81% was idelalisib-naïef. Onder de patiënten die niet eerder BTKi hadden gekregen resulteerde BTKi in ORR 84%; onder de patiënten die wel eerder BTKi hadden gekregen resulteerde BTKi in ORR 54%. PI3K-remmers resulteerden niet in duurzame remissies.
De onderzoekers concluderen dat na falen van venetoclax voor CLL BTKi een effectieve strategie kan zijn.

Summary: A large multinational study (326 patients) compared various treatments for CLL after discontinuation of venetoclax. For BTK inhibitor naïve patients, selection of covalently binding BTKis resulted in high ORR and durable remissions. PI3K inhibitors following venetoclax did not result in durable remissions.
