Er is geen
duidelijkheid over de waarde van complete macroscopische excisie (CME) of
incomplete macroscopische excisie (IME) van de primaire tumor na
inductietherapie voor stadium 4 hoog-risico neuroblastoom. De HR-NBL1 studie
van de International Society of Pediatric
Oncology Europe Neuroblastoma Group heeft deze waarde onderzocht. Dr. Ruth
Ladenstein (Medische Universiteit van Wenen) en collega’s publiceren de studie in het Journal of Clinical Oncology.1
Tusssen
begin 2002 en eind 2015 includeerde de studie 1531 patiënten die
inductietherapie zonder progressie hadden voltooid, hoge-dosering therapie
konden verdragen, en voorafgaand aan de inductie geen excisie hadden ondergaan.
Naar oordeel van de chirurg werd in 1172 patiënten CME (77%) uitgevoerd en in
359 IME (23%). De chirurgische mortaliteit bedroeg 0,46% (n=7), ernstige
operatiecomplicaties werden gezien in 142 patiënten (9,7%), en nefrectomie werd
uitgevoerd in 124 patiënten (8,8%). De patiënten kregen lokale radiotherapie
(21 Gy).
De mediane
observatietijd was 6,1 jaar. De vijf-jaars gebeurtenisvrije overleving was 40 ±
1% in de CME-groep versus 33 ±3% in de IME-groep (p<0,001). De vijf-jaars overall survival was 45 ± 2% in de
CME-groep versus 37 ± 3% in de IME-groep (p=0,004). De cumulatieve incidentie
van lokale progressie was significant lager na CME (17 ± 1%) dan na IME (30 ±
2%; p<0,001). Ook in de groep patiënten die immuuntherapie kregen waren de
uitkomsten significant beter na CME dan na IME.
De
onderzoekers concluderen dat in patiënten met stadium 4 hoog-risico
neuroblastoom na respons op inductietherapie, CME van de primaire tumor
geassocieerd was met betere overleving en lokale controle.

Summary: A study of the International Society of Pediatric Oncology Europe
Neuroblastoma Group found that in patients with stage 4 high-risk neuroblastoma
who had responded to induction therapy, complete macroscopic excision of the
primary tumor was associated with improved survival and local control after
high-dose therapy, local radiotherapy, and immunotherapy.
