Patiënten
met metastatisch retinoblastoom hebben een slechte prognose met conventionele
chemotherapie en radiotherapie. De multinationale prospectieve ARET0321 studie
van de Children’s Oncology Group heeft meer intensieve therapie voor deze
patiënten geëvalueerd. Dr. Ira Dunkel (Memorial Sloan Kettering Cancer Center,
New York) en collega’s publiceren de studie in het Journal of Clinical Oncology.1
De studie
includeerde 57 patiënten met extra-oculair retinoblastoom. Patiënten met
stadium II of III (locoregionaal-gevorderd) retinoblastoom kregen vier cycli
chemotherapie, gevolgd door involved-field
radiotherapie. Patiënten met stadium IVa of IVb retinoblastoom kregen eveneens
vier cycli chemotherapie, in geval van partiële respons of beter gevolgd door
één cyclus hoge-dosering carboplatine, thiotepa, en etoposide met autologe
hematopoïetische stamcel support. In geval van residuele tumor na chemotherapie
kregen patiënten met stadium IVa of IVb radiotherapie. De gebeurtenisvrije
overleving na een jaar was 88,1% (90%-bti 66,6-96,2) onder patiënten met
stadium II of III ziekte; 82,6% (61,0-92,9) onder patiënten met stadium IVa
ziekte, en 28,6% (12,7-46,2) onder patiënten met stadium IVb ziekte. De
toxiciteit was aanzienlijk zoals verwacht; twee patiënten overleden gerelateerd
aan de behandeling.
De
onderzoekers concluderen dat intensieve multimodaliteitentherapie zeer
effectief was voor patienten met stadium II, III, en IVa retinoblastoom. Voor
stadium IVb is meer-effectieve therapie vereist.

multinational prospective ARET0321 trial by the Children’s Oncology Group evaluated intensive multimodality
therapy for metastatic retinoblastoma. This therapy was highly effective for
patients with regional extraocular retinoblastoma and stage IVa metastatic
retinoblastoma. Although the study met its aim for stage IVb, more effective
therapy is still required for patients with CNS involvement.
