Het is
denkbaar dat een combinatie van gerichte therapie en cytotoxische chemotherapie
met immuuntherapie kan resulteren in verbetering van uitkomsten van patiënten
met maligniteiten. De multinationale fase 3-studie IMpassion050 heeft
toevoeging van atezolizumab aan de neoadjuvante combinatie van pertuzumab-trastuzumab
(PH) en chemotherapie geëvalueerd. Prof. Jens Huober (Kantonsspital St. Gallen,
Zwitserland) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
IMpassion050
werd uitgevoerd in 73 centra in twaalf landen. De studie includeerde 454
patiënten met een primaire tumor groter dan 2 cm en histologisch bevestigde
positieve lymfeklierstatus (T2-4, N1-3, M0). De patiënten werden 1:1
gerandomiseerd naar neoadjuvant atezolizumab plus PH en chemotherapie (n=228)
of placebo plus PH en chemotherapie (n=226), na chirurgie gevolgd door atezolizumab
plus PH of placebo plus PH. De totale duur van HER2-gerichte therapie was één
jaar. Coprimaire eindpunten waren pCR in de ITT-populatie en pCR in de
PD-L1-positieve populatie (n=218).
De figuur toont de resultaten van de studie. In de ITT-populatie was het
pCR-percentage 62,4% in de atezolizumabgroep versus 62,7% in de placebogroep
(p=0,9551). In de PD-L1-positieve populatie was het pCR-percentage 64,2% in de
atezolizumabgroep versus 72,5% in de placebogroep (p=0,1846). In de PD-L1
negatieve populatie was het pCR-percentage 60,7% in de atezolizumabgroep versus
53,8% in de placebogroep. Graad 3-4 en ernstige adverse events waren meer frequent in de atezolizumabgroep dan in
de placebogroep. Er waren vijf graad 5 AEs waarvan twee werden toegeschreven
aan de studiebehandeling, alle in de atezolizumabgroep. Het veiligheidsprofiel
was consistent met dat van atezolizumab in andere combinatiestudies.
De
onderzoekers concluderen dat toevoegen van atezolizumab aan neoadjuvant PH plus
chemotherapie niet resulteerde in verbetering van het pCR-percentage.

Summary: The
multinational phase 3 IMpassion050 trial found that addition of atezolizumab to
neoadjuvant anti-HER2 therapy and chemotherapy for early stage HER2-positive
breast cancer did not increase pCR rates in the ITT or PD-L1-positive
populations.
