De standaard-behandeling voor lokaal-gevorderd squameus celcarcinoom van de slokdarm (ESCC) is neoadjuvante (chemo)radiotherapie gevolgd door chirurgie, terwijl definitieve chemoradiotherapie de standaard-behandeling is voor niet-resectabel ESCC. Een fase 2-studie van Sun Yat-sen Universiteit (Guangzhou, China) heeft neoadjuvant paclitaxel plus cisplatine en 5-fluorouracil (TPF) voor borderline resectabel (BR-ESCC) geëvalueerd. Dr. Hong Yang en collega’s publiceren drie-jaars uitkomsten van de studie in Annals of Surgical Oncology.¹




De figuur laat de resultaten van NEOCRTEC-1601 zien. De studie includeerde 47 patiënten, van wie 27 (57,4%) na de neoadjuvante chemotherapie chirurgie ondergingen. R0-resectie kon worden uitgevoerd in 25 patiënten (53,2%). Pathologisch complete respons werd gezien in vier patiënten (8,5%). De mediane follow-up was 44,8 maanden (range 3,4-74,6). De mediane PFS onder alle patiënten was 38,7 maanden (95%-bti 23,5-53,9) en de mediane OS was 41,9 maanden (18,6-65,3). Het drie-jaars OS-percentage was 54,4% onder alle patiënten en 65,4% onder de patiënten de R0-resectie hadden ondergaan.

De onderzoekers concluderen dat onder patiënten met BR-ESCC, neoadjuvante chemotherapie gevolgd door chirurgie feasible is en tot gunstige overlevingsresultaten leidt.
