Tumorinfiltrerende
lymfocyten (TILs) zijn geassocieerd met profijt van trastuzumab en
chemotherapie voor vroeg-stadium HER2-positief mammacarcinoom (BC). Er is geen
duidelijkheid over de predictieve rol van TILs, TIL-subsets, en andere
immuuncellen in patiënten die chemotherapievrij lapatinib plus trastuzumab (LT)
krijgen. Een analyse van de TBCRC006-studie (twaalf weken neoadjuvant LT, plus
endocriene therapie voor ER-positieve tumoren) heeft deze rol onderzocht. Dr. Rachel Schiff (Baylor College of

De
onderzoekers scoorden in H&E-gekleurde slides stromale (s-)TILs in
pre-treatment biopten van 59 patiënten. Een drempelwaarde van 60% werd
gehanteerd voor het onderscheid van lymfocyt-predominant BC en niet-LPBC. In
FFPE-slides van 33 patiënten werden met multiplexed
immunofluoresence (m-IF) kleuring immuuninfiltraten gekarakteriseerd (CD4,
CD8, CD20, CD68, en FoxP3).
De analyse
liet zien dat het percentage tumoren met pCR numeriek hoger was voor LPBC dan
voor niet-LPBC (50% versus 18%; p=0,057). Unsupervised hiërarchische clustering
van de vijf immuunmarkers identificeerde twee clusters (cluster 1 met lage
versus cluster 2 met hoge CD4+, CD8+, CD20+ s-TILs en CD20+ intratumorale TILs)
met verschillende responsen op LT (cluster 1 pCR 7% versus cluster 2 pCR 50%;
p=0,01). In multivariate analyse was cluster 2 onafhankelijk geassocieerd met
hoger percentage pCR (p=0,03). Analyse van afzonderlijke immuun-subpopulaties
liet significante associatie zien van pCR met baseline hogere infiltratie door
s-CD4, intratumorale CD4, en intratumorale CD20+ TILs.
De
onderzoekers concluderen dat LPBC geassocieerd was met marginaal hogere pCR dan
niet-LPBC. Kwantitatieve beoordeling van immuuninfiltraten met m-IF was feasible.

Summary: An analysis of the neoadjuvant
TBCRC006 trial (12-week lapatinib plus trastuzumab for HER2-positive breast
cancer) found that the pCR rate was numerically higher in lymphocyte-predominant
breast cancer than in non-LPBC (50% versus 19%; p= 0.057). Quantitative
assessment of the immune infiltrate by multiplexed immunofluoresence was
feasible.
