Van de
combinatie van nivolumab (nivo) plus ipilimumab (ipi) is werkzaamheid gezien
voor hersenmetastasen van melanoom (MBM). In de meeste studies van deze
werkzaamheid waren patiënten met symptomatische MBM en/of eerdere lokale
hersenradiotherapie geëxcludeerd. Een multicenterstudie in Duitsland heeft de
werkzaamheid onderzocht van nivo plus ipi met of zonder lokale therapie voor
symptomatische en voor asymptomatische MBM, ongeacht de lijn van behandeling. Dr.
Theresa Amaral (Eberhard Karls Universität van Tübingen) en collega’s
publiceren de studie online in het Journal for
ImmunoTherapy of Cancer.1
De studie,
uitgevoerd in 23 centra, includeerde 380 patiënten met MBM, van wie 31%
symptomatische MBM hadden. De mediane follow-up was 18 maanden; de twee-jaars overall survival was 41% en de
drie-jaars OS was 30%. Onafhankelijke OS-voorspellende factoren waren serumniveaus van LDH en S100B, aantal MBM, en
ECOG performance status. De OS verschilde niet tussen patiënten die in eerste
lijn hetzij BRAF- of MEK-remmer of nivo plus ipi kregen, noch in de groep met BRAFV600-gemuteerd melanoom noch in de
groep met BRAF-wildtype melanoom.
Toevoegen van lokale therapie (chirugie of stereotactische radiochirurgie)
resulteerde in verbetering van de OS, ongeacht het tijdstip van lokale therapie.
De
onderzoekers concluderen dat nivo plus ipi, met name in combinatie met lokale
therapie, resulteerde in verbetering van de OS van patiënten met symptomatische
en van patiënten met asymptomatische MBM.
