BK-virus geassocieerde hemorragische cystitis (BKV-HC) is een niet-ongebruikelijke complicatie na allogene hematopoïetische stamceltransplantatie (alloHCT). BKV-HC kan resulteren in pijnlijke hematurie, urinaire obstructie, en nierdysfunctie. Een prospectieve studie van MD Anderson Cancer Center (Houston TX) heeft feasibiliteit, veiligheid, en werkzaamheid geïnventariseerd van toediening van BKV-specifieke cytotoxische T-lymfocyten (CTLs) afkomstig van most closely HLA-gematchte gezonde donoren als therapie voor BKV-HC na alloHCT. Dr. Katayoun Rezvani en collega’s publiceren de studie in het Journal of Clinical Oncology.¹

De studie includeerde 59 patiënten die BKV-HC ontwikkelden na alloHCT. De CTLs waren verkregen uit 26 donoren, en voor off-the-shelf gebruik opgeslagen in een CTL-bank. Infusie van CTLs resulteerde in respons in 40 van 59 evalueerbare patiënten (67,7%) na 14 dagen en in 40 van 49 evalueerbare patiënten (81,6%) na 45 dagen. Alle responsen bleven behouden. Er waren geen gevallen van graad 3 of 4 GVHD, graft failure, of infusie-gerelateerde toxiciteiten. BKV-CTLs werden gedetecteerd in bloedmonsters van de patiënten tot drie maanden na de infusie.

De onderzoekers concluderen dat off-the-shelf BKV-CTLs een veilige en effectieve behandeling vormen voor BKV-HC na alloHCT.
