De
PARP-remmer olaparib is goedgekeurd voor behandeling van HER2-negatief
metastatich mammacarcinoom (MBC) in dragers van kiemlijn (g) mutaties in BRCA1/2. De multicenter fase 2-studie
TBCRC 048 in de Verenigde Staten evalueerde olaparib voor MBC met somatische
(s) BRCA1/2-mutaties of g/s-mutaties
in andere genen die betrokken zijn bij homologe recombinatie (HR). Dr. Nadine
Tung (Beth Deaconess Medical Center, Boston MA) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 54 MBC-patiënten, onder wie 76% met ER-positieve HER2-negatieve
ziekte. De patiënten werden geïncludeerd in twee cohorten. Cohort 1 bestond uit
27 patiënten met kiemlijnmutaties in non-BRCA1/2
HR-gerelateerde genen, en cohort 2 bestond uit 27 patiënten met somatische
mutaties in deze genen of BRCA1/2. De
patiënten kregen oraal olaparib 300 mg tweemaal daags tot progressie optrad.
Het primaire eindpunt van de studie was percentage patiënten met objectieve
respons.
De ORR was
33% in cohort 1 (90%-bti 19-51) en 31% in cohort 2 (90%-bti 15-49). Bevestigde
responsen werden alleen gezien met gPALB2
(ORR 82%) en sBRCA1/2 (ORR 50%) maar
niet met ATM of CHEK2. De mediane progressievrije overleving was 13,3 maanden
(90%-bti 12- NA) voor gPALB2 en 6,3
maanden (90%-bti 4,4-NA) voor sBRCA1/2.
De
onderzoekers concluderen dat olaparib een werkzame behandeling was voor MBC met
gPALB2- of sBRCA1/2-mutaties.
