De
multinationale fase 3-studie KEYNOTE-177 randomiseerde patiënten met
microsatellite-instability-high (MSI-H) of mismatch-repair-deficient (dMMR)
metastatisch colorectaalcarcinoom (mCRC) naar pembrolizumab of chemotherapie.
In 2020 is gepubliceerd dat met mediaan 32,4 maanden follow-up de progressievrije
overleving significant langer was met pembrolizumab dan met chemotherapie. Prof.
Luis Diaz (Memorial Sloan Kettering Cancer Center, New York) en collega’s publiceren nu in The Lancet Oncology
de overall survival analyse van de
studie.1
KEYNOTE-177
werd uitgevoerd in 193 academische centra in 23 landen. De studie includeerde
volwassen patiënten met een ECOG performance status 0 of 1 en niet-eerder
behandeld MSI-H/dMMR mCRC. De patiënten werden gerandomiseerd naar
pembrolizumab 200 mg iedere drie weken (n=153) of chemotherapie iedere twee
weken (n=154). Chemotherapie was investigator’s
choice uit mFOLFOX6 of FOLFIRI met of zonder bevacizumab. Na progressie
konden patiënten uit de chemotherapiegroep overgaan op pembrolizumab.
De nu
gepubliceerde analyse werd uitgevoerd na mediaan 44,5 maanden follow-up (IQR
39,7-49,8). Er waren 93 patiënten uit de chemotherapiegroep overgegaan op
pembrolizumab (n=56) of andere anti-PD-(L)1 therapie (n=37). De mediane OS was
niet bereikt met pembrolizumab versus 36,7 maanden met chemotherapie (HR 0,74;
p=0,036) waarmee niet voldaan werd aan het vooraf-gespecificeerd criterium voor
superioriteit van pembrolizumab over chemotherapie (een α van 0,025). De
mediane PFS was 16,5 maanden met pembrolizumab versus 8,2 maanden met chemotherapie
(HR 0,59; 95%-bti 0,45-0,79). Graad 3 of hoger treatment-related adverse events waren minder frequent met
pembrolizumab dan met chemotherapie (22% versus 66%). Eén patiënt in de
chemotherapiegroep overleed aan darmperforatie, toegeschreven aan de chemotherapie.
De
onderzoekers concluderen dat pembrolizumab vergeleken met chemotherapie ook op
lange-termijn duurzame antitumor-activiteit had en minder TRAEs veroorzaakte
onder patiënten met MSI-H/dMMR mCRC. De OS was numeriek maar niet statistisch
significant beter in de pembrolizumabgroep dan in de chemotherapiegroep.
Pembrolizumab was een werkzame eerstelijns therapie voor patiënten met
MSI-H/dMMR mCRC.

Summary: Final analysis of KEYNOTE-177 showed that, among patients with
MSI-H/dMMR mCRC, pembrolizumab had durable antitumor activity and fewer TRAEs
compared with chemotherapy. The OS was numerically but not statistically
significantly better with pembrolizumab than with chemotherapy. The authors
conclude that the findings support pembrolizumab as an efficacious first-line
therapy in patients with MSI-H/dMMR mCRC.
