De multinationale fase 3-studie KEYNOTE-775 randomiseerde 827 patiënten met eerder-behandeld gevorderd, recidiverend, of metastatisch endometriumcarcinoom (EC) naar lenvatinib plus pembrolizumab (n=411) of chemotherapy of the treating physician’s choice (docorubicine of paclitaxel; n=416). Vorig jaar is gepubliceerd dat de progressievrije overleving significant beter was met lenvatinib plus pembrolizumab dan met chemotherapie.Dr. Vicky Makker (Memorial Sloan Kettering Cancer Center, New York) en collega’s publiceren nu de finale analyse van de studie in het Journal of Clinical Oncology.¹

Onder de 827 geïncludeerde patiënten hadden 697 mismatch repair-proficient (pMMR)-ziekte en 130 dMMR-ziekte. De mediane follow-up was 18,7 maanden in de lenvatinib-pembrolizumabgroep en 12,2 maanden in de chemotherapiegroep. De figuur laat zien dat de overall survival en progressievrije overleving significant beter waren met lenvatinib plus pembrolizumab dan met chemotherapie, zowel in de pMMR-populatie (A,C) als onder alle patiënten (B,D). Betere uitkomsten met lenvatinib plus pembrolizumab versus chemotherapie werden gezien in alle onderscheiden subgroepen. Er waren geen nieuwe veiligheidssignalen.

De onderzoekers concluderen dat onder patiënten met eerder-behandeld gevorderd EC, behandeling met lenvatinib plus pembrolizumab ook met langere follow-up resulteerde in betere OS en PFS dan chemotherapie, met manageable veiligheid.

Lenvatinib plus pembrolizumab resulted in longer OS and PFS than chemotherapy, both in the pMMR population and among all patients.
