Moh’s
micrografische chirurgie (MMS) wordt in toenemende mate toegepast voor de
behandeling van melanoom in situ. De richtlijnen voor invasief melanoom zonder
nodale metastasen bevelen brede-marge excisie aan (WME), maar ook voor deze
ziekte wint MMS aan populariteit, vooral in vroege stadia. Een analyse van de National Cancer Database heeft overall survival geïnventariseerd na MMS
versus WME voor stadium I (AJCC-8) melanoom. Prof. Michael Girardi (Yale School
of Medicine, New Haven CT) en collega’s publiceren de analyse online in JAMA Dermatology.1
In de NCDB
identificeerden de onderzoekers 70.319 patiënten (52,3% mannen en 47,7%
vrouwen, mediane leeftijd 57 jaar) die tussen begin 2004 en eind 2014 chirurgie
ondergingen voor stadium I melanoom. De WME-groep bestond uit 67.085 patiënten
en de MMS-groep uit 3234 patiënten. Na controle voor klinische en tumor-factoren
was MMS vergeleken met WME geassocieerd met licht betere OS (HR 0,86; 95%-bti
0,76-0,97), een conclusie die werd bevestigd in voor propensity score gematchte
analyse (HR 0,82; 95%-bti 0,68-0,98). MMS werd meer frequent toegepast in
academische dan in niet-academische centra (OR 2,03; 95%-bti 1,88-2,18).
De onderzoekers
concluderen dat MMS een verantwoord alternatief voor WME kan zijn voor stadium
I melanoom in geselecteerde patiënten.

Database found that Mohs surgery may provide an alternative approach to
traditional wide margin excision for appropriately selected cases of AJCC-8
stage I melanoma, without compromising patient survival.
