Richtlijnen
bevelen endocriene behandeling aan voor HR-positief HER2-negatief metastatisch
mammacarcinoom (MBC) in zowel premenopauzale als postmenopauzale patiënten. In
de klinische praktijk krijgen veel patiënten echter chemotherapie. De
gerandomiseerde fase 2-studie KCSG-BR15-10 in veertien centra in Zuid-Korea
vergeleek palbociclib pus endocriene therapie en een GnRH-agonist versus
capecitabine chemotherapie voor HR-positief HER2-negatief MBC in premenopauzale
vrouwen. Prof. Yeon Hee Park (Samsung Medisch Centrum, Seoel) en collega’s
publiceren de studie online in The Lancet Oncology.1
De studie
includeerde patiënten in de leeftijd van 19 jaar of ouder met HR-positief
HER2-negatief mammacarcinoom met recidief of progressie tijdens
tamoxifen-behandeling, een ECOG performance status 2 of beter, en ten hoogste
één eerdere lijn chemotherapie voor metastatische ziekte. Ze werden
gerandomiseerd naar palbociclib plus exemestaan en leuprolide (n=92) of
capecitabine (n=86). Het primaire eindpunt van de studie was progressievrije
overleving. De mediane follow-up was 17 maanden (IQR 9-22). De mediane PFS was
20,1 maanden in de palbociclib plus exemestaan-groep versus 14,4 maanden in de
capecitabinegroep (HR 0,659; p=0,0235). Graad 3 of 4 neutropenie was meer
frequent in de palbociclib plus exemestaan-groep dan in de capecitabinegroep
(75% van de patiënten versus 16%). Ernstige TRAEs werden gerapporteerd voor 2%
in de palbociclib plus exemestaan-groep en 17% in de capecitabinegroep.
Follow-up voor overall survival is ongoing.
De
onderzoekers concluderen dat palbociclib plus endocriene therapie plus
ovariumfunctiesuppressie vergeleken met capecitabine resulteerde in langere
progressievrije overleving van premenopauzale patiënten met HR-positief
HER2-negatief MBC.

Korean multicenter randomized phase 2 study KCSG-BR15-10 compared palbociclib
plus exemestane and leuprolide versus capecitabine for premenopausal
HR-positive, HER2-negative metastatic breast cancer. The study found better PFS
in the palbociclib plus exemestane group than in the capecitabine group (median
20.1 months versus 14.4 months; p=0.0235). Grade 3 or 4 neutropenia was more
common in the palbociclib plus exemestane group (75% versus 16%).
Treatment-related serious events were noted in 2% of patients in the
palbociclib plus exemestane group and 17% of patients in the capecitabine
group.
