De
multinationale fase 3-studie KEYNOTE-048 randomiseerde patiënten met
recidiverend of metastatisch squameus celcarcinoom van hoofd en hals (R/M
HNSCC) 1:1:1 naar pembrolizumab, pembrolizumab-chemotherapie, of
cetuximab-chemotherapie. In 2019 is gepubliceerd dat de overall survival
met pembrolizumab monotherapie langer was dan met cetuximab-chemotherapie onder
de patiënten met PD-L1 CPS 20 of hoger en 1 of hoger , en dat de OS met
pembrolizumab-chemotherapie langer was dan met cetuximab-chemotherapie onder
alle gerandomiseerde patiënten. Prof. Kevin Harrington (Institute of Cancer
Research, London UK) en collega’s publiceren in het Journal of Clinical
Oncology lange-termijn resultaten van de studie, na mediaan 45,0 maanden
follow-up (IQR 41,0-49,2).1
KEYNOTE-048
werd uitgevoerd in 200 centra in 37 landen. Deze figuur laat zien dat ook met langere follow-up het OS-profijt van
pembrolizumab en pembrolizumab-chemotherapie versus cetuximab-chemotherapie
bleef bestaan. Progressievrije overleving op een volgende therapie (PFS2) was
langer in de pembrolizumabgroep dan in de cetuximab-chemotherapiegroep onder
patiënten met PD-L1 CPS 20 of hoger (HR 0,64; 95%-bti 0,48-0,84) en PD-L1 CPS 1
of hoger (0,79; 0,66-0,75), en PFS2 was langer met pembrolizumab-chemotherapie
dan met cetuximab-chemotherapie onder patiënten met PD-L1 CPS 20 of hoger
(0,64; 0,48-0,86), PD-L1 CPS 1 of hoger (0,66;
0,55-0,81), en onder alle patiënten (0,73; 0,61-0,88).
De
onderzoekers concluderen dat met langere follow-up pembrolizumab en
pembrolizumab-chemotherapie voor R/M HNSCC resulteerden in overlevingsvoordeel
vergeleken met cetuximab-chemotherapie, en dat patiënten ook betere PFS2 hadden
na eerdere pembrolizumab-gebaseerde therapie.

Summary: Updated results of the KEYNOTE-048
trial show that with a 4-year follow-up, first-line pembrolizumab and
pembrolizumab-chemotherapy continued to demonstrate survival benefit versus
cetuximab-chemotherapy in recurrent/metastatic head and neck squamous cell
carcinoma. Patients responded well to subsequent treatment after
pembrolizumab-based therapy.
