In
verscheidene typen tumoren is gezien dat hoge microsatellietinstabiliteit
(MSI-H) tumoren in hoge mate vatbaar maakt voor immuuntherapie. Een post-hoc
analyse van drie klinische studies (fase 2 KEYNOTE-059; fase 3 KEYNOTE-061 en
-062) heeft de associatie van MSI-H met werkzaamheid van pembrolizumab voor
maag- of maag-slokdarmovergang (G/GEJ)-carcinoom onderzocht. Dr. Joseph Chao (City of Hope Comprehensive
Cancer Center, Duarte CA) en collega’s publiceren de analyse in JAMA Oncology.1
De eenarmige
studie KEYNOTE-059 evalueerde derde- of laterelijns pembrolizumab voor
gevorderd G/GEJ-carcinoom; KEYNOTE-061 randomiseerde patiënten met gevorderd
G/GEJ-carcinoom naar tweedelijns chemotherapie of pembrolizumab; en KEYNOTE-061
randomiseerde patiënten met gevorderd G/GEJ-carcinoom naar eerstelijns
pembrolizumab of chemotherapie of pembrolizumab plus chemotherapie. MSI-H
tumoren werden gezien in 4,0% van de KEYNOTE-059 patiënten, 5,3% van de
KEYNOTE-061 patiënten, en 7,3% van de KEYNOTE-062 patiënten. Terwijl onder alle
patiënten in de studies weinig verschil was in respons of progressievrije
overleving tussen de chemotherapie- en de pembrolizumabgroepen, waren onder de
patiënten met MSI-H tumoren de ORR en PFS significant beter met pembrolizumab
(al of niet met chemotherapie) dan met alleen chemotherapie.
De
onderzoekers concluderen dat onder patiënten met gevorderd G/GEJ-carcinoom
MSI-H ongeacht de lijn van behandeling een biomarker voor profijt van
pembrolizumab.

Summary: Post hoc analysis of three prospective
studies found that pembrolizumab with or without chemotherapy was more
beneficial than chemotherapy alone for the treatment of patients who had
microsatellite instability-high advanced or metastatic gastric or
gastroesophageal junction cancer, across all lines of therapy. MSI-H status may
be a biomarker for pembrolizumab therapy among patients with advanced G/GEJ
cancer regardless of the line of therapy.
