De
multinationale fase 3-studie PROfound randomiseerde patiënten met metastatisch
castratieresistent prostaatcarcinoom (mCRPC) na progressie op een next-generation hormonal agent (NHA) en
veranderingen in BRCA1, BRCA2, en/of ATM naar olaparib monotherapie of physician’s choice uit abirateron of enzalutamide
(controle). In 2020 is gepubliceerd dat de radiografische progressievrije overleving (rPFS) en de overall survival (OS) significant beter
waren met olaparib dan met controle. Dr. Joaquin Mateo (Vall d’Hebron
Universiteitsziekenhuis, Barcelona) en collega’s publiceren in het Journal of Clinical Oncology resultaten van de studie in de
subgroep van patiënten met BRCA1-
en/of BRCA2-veranderingen.1
De analyse
includeerde 160 patiënten. De figuur laat zien dat ook in de
subgroep met BRCA1/2-veranderingen de
rPFS (mediaan 9,3 versus 3,0 maanden; HR 0,22; 95%-bti 0,15-0,32) en OS
(mediaan 20,1 versus 14,4 maanden; 0,63; 0,42-0,95) significant beter waren met
olaparib dan met controle. Het rPFS-profijt met olaparib versus controle werd
gezien in alle zygositeit-subgroepen. Patiënten met BRCA2 homozygote deleties hadden extra lange rPFS met olaparib
(mediaan 16,6 maanden).
De
onderzoekers concluderen dat in alle geanalyseerde subgroepen olaparib
resulteerde in significant betere uitkomsten dan abirateron of enzalutamide.
