Een
multicenter studie in China heeft anti-thymocyt globuline (ATG) plus
lage-dosering post-transplant cyclofosfamide (PTCy; 14,5 mg/kg op dagen drie en
vier) vergeleken met alleen ATG voor de preventie van graft-versus-host disease (GVHD) na haploïdentieke transplantatie.
Prof. Xiao-Jun Huang (Volksziekenhuis van de Peking Universiteit, Beijing) en
collega’s publiceren de studie online in het Journal of
Hematology & Oncology.1 De studie includeerde 114 patiënten
in het ATG-PTCy cohort, en vergeleek de uitkomsten met die van een contemporair
cohort van 125 patiënten die alleen ATG kregen.
De
transplantatie resulteerde in succesvolle engraftment binnen dertig dagen in
alle patiënten op één na (in het alleen-ATG cohort). De figuur laat zien dat het ATG-PTCy cohort
vergeleken met het alleen-ATG cohort de incidentie van acute en chronische GVHD
en nonrelapse mortaliteit lager was. De incidentie van relapse was gelijk in
beide cohorten, en de overall survival
was niet-significant langer in het ATG-PTCy cohort. De GVHD-vrije relapse-vrije
overleving was significant langer in het ATG-PTCy cohort.
De
onderzoekers dat toevoegen van lage-dosering cyclofosfamide aan ATG voor de
preventie van GVHD na haploïdentieke transplantatie resulteerde in verbetering
van de uitkomsten.

Summary: A multicenter study in China multicenter study in China compared
anti-thymocyte globulin plus low-dose post-transplant cyclophosphamide plus
(ATG-PTCy) versus ATG alone for prevention of GVHD after haploidentical HCT. The ATG-PTCy cohort had less acute
and chronic GVHD, less nonrelapse mortality, similar relapse, similar overall
survival, and longer GVHD-free, relapse-free survival compared tot the ATG only
cohort.
