De Consensus Molecular Subgroup
(CMS)-indeling onderscheidt colorectaalcarcinoom in vier groepen op basis van
genexpressiesignaturen. De relevantie van deze indeling voor de behandeling is
nog niet goed gedefinieerd. Een exploratieve analyse van resultaten van de FIRE3-studie
heeft de prognostische en predictieve betekenis van CMSs van metastatisch CRC
onderzocht. Prof. Sebastian Stintzing (Charité Universitätsmedizin Berlijn) en
collega’s publiceren de analyse online in Annals of Oncology.1
FIRE3
vergeleek eerstelijns FOLFIRI plus cetuximab versus FOLFIRI plus bevacizumab
voor KRAS exon 2 wildtype mCRC. De
CMS-categorie kon worden bepaald in tumoren van 438 patiënten: 14% was CMS1,
37% was CMS2, 15% was CMS3, en 34% was CMS4. Onder de RAS-wildtype tumoren (n=315) was de verdeling 12% CMS1, 41% CMS2,
11% CMS3, en 34% CMS4. De CMS-distributie in rechts- versus linkszijdige
tumoren was CMS1 27% versus 11%, CMS2 28% versus 45%, CMS3 10% versus 12%, en
CMS4 35% versus 32%. Onafhankelijk van de behandeling was CMS een sterk
prognostische factor voor ORR (p=0,051), PFS (p<0,001), en OS (p<0,001).
Binnen de RAS-wildtype populatie was in CMS4 de OS significant beter in de
FOLFIRI-cetuximab arm dan in de FOLFIRI-bevacizumab arm. In CMS3 was er een
trend van betere OS met FOLFIRI-cetuximab dan met FOLFIRI-bevacizumab. In CMS1
en CMS2 had de behandelarm geen invloed op de OS.
De
onderzoekers concluderen dat de CMS-categorie prognostisch is in mCRC. De
langere OS met FOLFIRI-cetixumab versus FOLFIRI-bevacizumab in FIRE3 werd
gedreven door CMS3 en CMS4.

Summary: An exploratory analysis of results of
the FIRE3 study found that the Consensus Molecular Subgroup classification is
prognostic for mCRC. Prolonged OS induced by FOLFIRI plus cetuximab compared to
FOLFIRI plus bevacizumab appeared to be driven by CMS3 and CMS4. There is no
direct impact of CMS classification on clinical decision-making.
