De Immunoscore (IS) is ontwikkeld voor prognostische stratificatie van patiënten met stadium I tot en met III coloncarcinoom (CC). De score is gebaseerd op dichtheid van CD3+ en CD8+ T-cellen in de tumoren en invasieve marges, die immuunhistochemisch worden bepaald en gekwantificeerd met digitale pathologie. De prognostische en predictieve waarde van IS is onderzocht in de prospectieve IDEA France cohortstudie. Prof. Franck Pagès (Hôpital Européen Georges Pompidou, Parijs) en collega’s publiceren de analyse online in Annals of Oncology.¹

De studie vergeleek drie versus zes maanden oxaliplatine-gebaseerde adjuvante chemotherapie voor stadium III CC. De nu gepubliceerde analyse includeerde monsters van 1322 patiënten (43,6% IS-laag; 47,0% IS-intermediair; 9,4% IS-hoog). De prognostische waarde van IS bleek uit de analyse dat patiënten met IS-laag hoger risico van recidief of overlijden hadden dan patiënten met IS-intermediair/hoog (HR 1,54; p=0,0001). De drie-jaars DFS was 66,8% in de groep met IS-laag versus 77,1% in de groep met IS-intermediair/hoog. In multivariate analyse na correctie voor geslacht, histologische graad, T/N-stadium, en microsatellietinstabiliteit, bleef IS significant geassocieerd met DFS (p=0,003). De predictieve waarde van IS bleek uit de waarneming dat voor de mFOLFOX6-behandelde patiënten (ruim 90% van het cohort) zes maanden behandeling geassocieerd was met significant betere DFS dan drie maanden behandeling onder de patiënten met IS-intermediair/hoog (HR 0,53; p=0,004), maar niet onder patiënten met IS-laag.

De onderzoekers concluderen dat deze resultaten de prognostische en predictieve waarde van IS voor oxaliplatine-behandeld stadium III CC bevestigden.
