Voor de meerderheid van de patiënten met acute myeloïde leukemie (AML) is allogene stamceltransplantatie (SCT) in eerste complete remissie (CR) de geprefereerde behandeling. Het is niet duidelijk of het aantal inductiecycli dat vereist is om CR te bereiken prognostische impact heeft. Een retrospectieve multicenterstudie van de European Society for Blood and Marrow Transplantation heeft uitkomsten vergeleken van patiënten met CR na één versus twee inductiecycli. De studie includeerde volwassen AML-patiënten die in eerste CR SCT ondergingen van hetzij volledig gematchte sibling of 10/10 of 9/19 HLA-gematchte niet-verwante donor. Onder de 4995 geïncludeerde patiënten waren er 3839 (77%) die CR bereikten na één inductiecyclus en 1116 (23%) die twee inductiecycli nodig hadden om tot CR te komen. De figuur laat zien dat deze laatste groep patiënten slechtere incidentie van relapse, leukemievrije overleving, en overall survival maar niet nonrelapse mortaliteit hadden dan patiënten die al na één inductiecyclus CR bereikten. In multivariate analyse was noodzaak van twee inductiecycli geassocieerd met verhoogd risico van relapse (HR 1,38; p=0,0003) en overlijden (1,27; p=0,002).

De onderzoekers concluderen dat initiële respons op chemotherapie ook in patiënten die SCT ondergingen prognostische relevantie behield.
